Each child will need a 2022 Release Form filled out in its entirety in order to to attend youth events at Applegate Christian Fellowship. This form will be kept on file for each subsequent 2022 youth event and can be found by clicking the pdf link below. Forms can be turned in by giving them to your Childs youth leader or by dropping them in the box below the flyer rack in the foyer.
